Unable to boot to multi-user mode due to failure of svc:/system/filesystem/usr:default

Hi Experts,

I posted this on another thread but with a different subject, I would like to move it here under a more descriptive subject.

One of our servers is unable to boot the server to multi-user mode. It loads the kernel and boot archive and boots, but i t complains "fsck: could not stat /dev/rdsk/c2t0d0s0: No such file or directory" at the fsck stage. thus/ file system was mounted read-only and other files systems listed in the /etc/vfstab were not mounted. And subsequently it enter maintenance mode. I booted the server to failsafe mode and ran fsck -F -y on all files systems,  but the iusse remained the same.
