Collection Plan is not Trigerred

We have collection plan defined and it is tied to a Operation completion transaction Triggered by activity. I have made the transaction (EAM Operation Completion) as mandatory.  The Work order has the same activity, but when I try to complete an operation in the WO, this is not triggering collection plan results entry.
Trying to find if i am missing something
