Getting Duplicate description value - dm_org_desc err in t_src_loc_tmpl table

We are getting Duplicate description value - dm_org_desc in t_src_loc_tmpl_err table which is preventing us to load location data,We deleted the Org from t_ep_organization table and  recrerated it ..The issue still present

please help
