User name on hold is incorrect

There is an normal Order Hold created via 'Create Hold Source' by user X.

However, when the orders are being put on a hold and user X wants to release this hold, the name of 'Held By' is user Y

So not the user who initially created the hold to begin with.
I cannot check if that user Y was the user who has booked the order for example, as in the mean time other activities have taken place & the record history doesn't show any trace of the user Y.

How come the wrong user name is in the 'Held By' ?
