Workflow to Send a Task is Firing Twice

Hello,

I have created a workflow on Activity When Modified Record Saved that will send a task.  It is working HOWEVER it is doing it twice.  This happened/happens every time I use the email or task option.  Any suggestion, tips, resolutions?
