Error launching forms after upgrade to 12.1.3

 Hi,

We have upgraded a system from 11.5.9 to 12.1.3 on Linux x64.
Everything went fine and the application is up and running.
However, each time we try to open any form we encounter the following error:

The requested URL /forms/frmservlet was not found on this server.

The following error appeared in the error_log of the 10g application server:

[Wed Oct  3 15:05:57 2012] [error] [client 192.168.0.2] [ecid: 1349269557:192.168.0.224:25593:0:13016,0] File does not exist: /oracle/appltest/inst/apps/TEST_orafin1/portal/forms/frmservlet

The directory in the message does not exist. There are no directories inside $INST_TOP/portal
I researched MOS and the web for solutions but found nothing useful.
