Index Out Of Range Error

I have a user that has created some Financial Reporting Books.  He is now having issues making any edits to them.  When he gets into one, the folder structure on the right side that allows him to add new documents is almost blank.  All he can see in the structure is the Root folder.   When he clicks on the folder he gets and Internal Server Error.  When he views details, it just says there was an issue communicating with the server.  I check the Server logs and find an entry in the FinancialReporting Out file that looks like this:
