Patch 3171663 error in generating form PAYWSDBT.fmx

When running the patch 3171663, there is message
The following Oracle Forms objects did not generate successfully:
pay forms/US PAYWSDBT.fmx
An error occurred while generating Oracle Forms files.

We continue as if it were successful, and the autopatch finish.

We then compile the form again, and there are errors
PL/SQL ERROR 49 at line xxx, column xx
bad bind variable

Please advise how to resolve the problem.

And we want to ask, after applying patch 3171663, what should be the release number?
'SELECT RELEASE_NAME FROM FND_PRODUCT_GROUPS' returns 11.5.9, which is same as before applying the patch.
