MS Word 2010 RTF, not seeing Format Picture for dummy image to enter Alt Text

I am using BI Publisher 11.1.1.5.0 on MS Word 2010.

I am not seeing Format Picture option to enter url under Alt Text when I right click on the image. I researched a lot to see why it is not showing Format Picture.

But if I insert an dummy image after the end of the template, then right click then I see Format Picture.

Can you pls tell me how to add dummy image within the template and see Format Picture to add the URL? Your help is very much appreciated.
