Numerous issues in Production 12.1.3 Upgrade despite DEV/UAT being problem free

I could go on at length but to keep it short I've just undergone the upgrade from hell.

Twice I managed to get into a situation where by the adpatch would not restart the 12.1 upgrade. This being well into the upgrade and already having run for many hours.

 I believe because adpatch bombed out with core dumps( segmentation faults) the restart files were left in a corrupted state. Basically the workers had failed yet they were still showing as running under adctrl. And Orcale Support were unable to help me correct the restart files/workers table and so the only course of action was a restore and restart.
