Charges Service Activities disappear

Testing in R12

I have created some forms personalizations to default the Business Process based on the Operating Unit. After creating these when I enter a Service activity, save and submit charges the service activity clears out and therefore creates a blank sales order line type.

Any ideas would be appreciated.
