Requisition history comes with error 6512

 Dear All,Our user has raised a requisition which has been approved by all the approvers in the hierarchy. But when queried from forms, it is fond at the last but one approver. When approver is viewed through workflow, status shows error 6512. In forms the status is "in process". Can anyone help with solution.
