Order line workflow not assigned

Hi,

We're on R12.1.3

We're seeing an issue where order lines are not getting associated to line workflows and remaining in Booked status. This is not happening for all the order lines and types. I mean this issue is not reproducible at will and happens intermittently.

When tried to Actions ==>Progress order nothing happens as there are no activities exists/workflow exists.

Any thoughts please?
