Inactive Credit Card Records

Why does Oracle not let you know that you are creating multiple records when a credit card has been tagged as Inactive?  How can we remove the extra records?

We had a situation where a user made repeateed attempts to enter a credit card to no avila.  The steps taken were:

Enter the data

Click Apply

Record would appear on display

Click Save 

Record disappeared

No message of any kind

A Toad query indicated the record did exist, but was inactive.  Only one recrod wsa retrieved in the query.

Later I discovered the responsibility "Funds Capture Prcoess Manager" allowed me to activate the card.
