Inconsistent RAC one-node instance status

We are running RAC one-node for a dozen or so databases in our environment. 
OEM is 12c Cloud Control Release 1 with Patch Bundle installed.

Normal display behavior for RAC1 in OEM 12c is one instance up and one instance down.  Status displays properly for some of our databases, but not others.

Some display <sid>_1 and <sid>_2 both up (never the case) and others show <sid>_1, <sid>_2, and also <sid>.

We've removed and reinstalled the agent.  No changes.

Do I need to manually remove the targets.xml and rediscover the host nodes?

Am I barking up the wrong tree?  Is this a symptom of problems with my database parameters like local and remote listener?
