Read Audit showing incorrect values intermittently

Hi ,

Readt Audit Trail has been turned ON for the Account BC.  Upon selecting a record from the Find result then navigating to the Read Audit Items View, the Field Details reflects 0000-00-00 instead of the actual value.

When an Account record is saved using the UI, the same value is saved in the Database in S_ORG_EXT.  
Intermittent display of incorrect value in the Read Audit Trail view for this specific field .The DB also holds the value of 0000-00-00 in the S_AUDIT_READ table.

Any inputs to debug this issue?
