Not receiving requisition approval emails

Hello All.

We just upgraded to 12.1.3 and we are no longer receiving any email notifications for requisitions.  We do receive the online notifications.  The wf_notification status shows as Failed but I am not finding any hits that match any of the errors in the log.  Other emails are going out successfully.  Has anyone else had this problem as a result of an upgrade from 11.5.10?
