Generate Approval for Quote or Order not sending outbound Email

On the vanilla 8.1.1.x application, when you select the menu option "Generate Approval" for a Quote then an inbox record is created and a Quote Approval record is created. However no email is sent to the approver defined in the Admin Application>Approval Admin and "Quote Approval". I setup the signal "ApproveItem" with parameter Email = True as documented in the Knowledge Doc Id 475324.1.

 

 

In the log file the workflow "ISS Approval (Quote)" is executed and the step "Approval Bus. Service" is executed when the Quote passes the validation rule defined in the Admin Data Validation>Rule Set "Quote Approval". I setup the quote with a total value to pass this vanilla validation rule but there was no change to the application. The workflow runs the method CreateNewApprovalTasks documented in the bookshelf Siebel Order Management Guide for method CreateNewApprovalTasks at web address:
