EBS R12 - Printing Government pre-defined Forms (PDF format)

Dear All,

Using BI Publisher with EBS R12, I need to print Government pre-defined Forms (PDF format).

1. Has anybody worked on this kind of requirement. How we can create PDF template?

   Its an pre-defined form in PDF format, I need to fill the data and print it in E-BSuiness suite R12.

   Please share your experience.

2. Do we need to buy any specific plug-in for Acrobat to create Form Fields?
