PR is not getting approved/forward

Hi all,

User is making PR & forwarding to respective hierarchy for approval/forwarding. This changes the status of PR to ‘In Process’ successfully; but after  ‘Workflow Background Engine’ is run, the status is again changed to ‘Incomplete’ & no notification is sent to approver. Please guide.
