Modifier lines with end-dates still firing during order entry

We have four modifier lines on three separate modifiers, all of which are end-dated at the line level.  These discounts are all firing regardless of qualifiers and end-dates.  We have removed the percent discount to minimize the impact to the order line, but still searching for root cause.  We are currently on version 12.1.3.  Any suggestions?
