Patch 13636268 : Rel 12.1 : AP : After applying Patch 13636268:R12.AP.B, OE Forms fail to generate w

Hi there

After applying Patch 13636268:R12.AP.B ("ISRAELI GDF NOT APPEARING ON INVOICE WORKBENCH AFTER APPLYING PATCH 9241039")
the following three Order Management forms will not generate :

  OEXOEORD.fmx
  OEXOEVER.fmx
  OEXOETEL.fmx

In all three cases there are no errors in any Package or Trigger compilations in the Worker log, but the log entry for the form
ends with :

  FRM-31640: Submenu name not specified.Popup Menu Item FOLDER
  Form: OEXOEORD
  FRM-30085: Unable to adjust form for output.

  Generated    0 files successfully.
  Generated    0 files with non-fatal warnings.
  Generated    1 files with fatal errors.
