Second page for a PDF template report

I am not able to get a report with a two page PDF template  to print the second page. The second page contains instructions only.

I have learned that PDF 2 page printing worked in 11i via patch 8452335 - OPTIONAL PRINTING OF PAGES IN PDF TEMPLATES, but is not working in R12.1.3 after upgrade.

This is to request help with any available work around.
