init errno 48

 Our T200 threw a message today which is strange. We applied OS patches at 4PM and we got this message at 6:56. Upgrade from 147440-19 to 147440-25.

Nov 19 06:56:00 reg-glb03 genunix: [ID 923222 kern.warning] WARNING: exec(/sbin/init) failed with errno 48.
Nov 19 06:56:00 reg-glb03 genunix: [ID 404865 kern.warning] WARNING: failed to restart init(1M) (err=48): system reboot required

System got many zombie process from this point and we had to reboot the server to fix it.

Anyone seen this error before? What might the cause of this error ?
