Adding a new server to Oracle Grid

I am new to oracle grid control and need some pointers and directions to add a new server/database to an existing grid control.

Recently, I have updated a server(both os and oracle database) which was previous being monitored and adminsirated by an exsitng Oracle Grid Controll 11g infrastructure.  After I've updated the server, the Oracle GRid Control is saying it is not reachable.  I believe I need to install an agent and configure the server to the grid.  This is where I am now and hope you could help.
