Linking iProcurement Smart Forms with PO Line Types

Is there any way to link a PO Line Type to the Smart Form?

I've created several Smat Forms in iProcurement Catalogue Manager and I would like the Requisiton Line and Purchase Order Line Type to be generated based on the Smart Form used:

Can anyone help?
