How to view item attachment from a quality collection plan

Hi , I have created a new attachment category and assigned it ti item form

I Have attached a an attachment to a specific item using the document category defined

now I want to be able to open quality collection plan , enter the item and view the attachment

I tried using the Attachment Function screen from Application development with no sucsess

Can some one help me?
