11.5.10.2 : Error Applying patch 12807777 on forms/web Tier "An error occurred while performing the

Hi,

We have a 2 node setup. Concurrent/admin/db tier and forms/web tier I have downloaded and applied patch 12807777 to the concurrent/admin/db tier
but when I apply to the forms/we tier I get the message "An error occurred while performing the second half of mirrored copies".

The log file is showing
