Grid Control Server Migration

in Enterprise Manager Generic (MOSC) Question
 Hello Expert,
We have installed Oracle 12c grid control on (OMS & OMR) on the same server i.e. Linux Box, configured with agents for all Prod & Dev serevrs.  Please suggest the steps, document as I need to migrate OEM (OMS & OMS) to new server.
