Report is not generating with associated/uploaded Template and picking other template

 Hi All,
I am facing very weird issue where one of the report is not generating with uploaded template. Report is generating with some other template and not sure from where it's picking that template. This report is working fine in other environments and behaving like this in QA environment only.

I have tried re-registering the report at Siebel and BI side however that's didn't help. Bounced the BI server many times however not success.
