SYS.NAME_SECURITY

A user reported error

E03020003: Database Error.   It is unable to extend a ‘lob’ segment.

The database starts up just fine, but cannot connect remotely.

I see when querying tablespace size that The index, UNDOTBS1, Users, System and TEMP are either full or very close.  I've tried adding a datafile to the temp tablespace but get the error:

alter tablespace users add datafile '/u01/oracle/oradata/datafile/USER2.dbf' size 100m

ERROR at line 1:
ORA-00604: error occurred at recursive SQL level 1
ORA-04045: errors during recompilation/revalidation of SYS.NAME_SECURITY
ORA-04067: not executed, package body "SYS.NAME_SECURITY" does not exist
