R12.1.1 patches(11651775, 12913767,13890183) are failing

Oracle has provided us with four patches 10623245,11651775, 12913767,13890183.
But only patch 10623245 is getting successfully migrated on our quality instance. Whereas for other three patches(11651775, 12913767,13890183) we are getting the following errors.
1) 10623245:Success

2) 11651775: Failed
Fmx not generated INVSDOIO.fmb
