Using <?start@last-page-first:body?> and different headers

The client I am working for has a requirement to display the totals at the bottom of the page if it can fit on one page or display on the bottom of the last page with a different header on all pages but the first page.  I can not get both of these to work.  When I used start@last-page-first the totaling works correctly and displays at the bottom but I still get the same first page header on every page even with Different first page selected on the second MS Word page header section. 

I am using subtemplates and calling the subtemplate header in the MS Word header.  I had to use subtemplates since I ran into an issue when I used long tables in the MS Word header and used my watermark it would duplicate the image.
