instance terminated

Hello All !
i have rac database of version 11.2.0.3 with two nodes. on 2nd node the instance is terminated. i have check the alert log file and getting the following errors:

LMON (ospid: 10384) waits for event 'control file sequential read' for 0 secs.
GES: System Load is HIGH.
GES: Current load is 3.12 and high load threshold is 0.31
LMON (ospid: 10384) waits for event 'control file sequential read' for 5 secs.
ERROR: LMON is not healthy and has no heartbeat.
ERROR: LMD0 (ospid: 22112) is terminating the instance.
LMD0 (ospid: 22112): terminating the instance due to error 482

opidrv aborting process S000 ospid (6744_47880) due to error ORA-482
