Patch 12784183 :Error while applying patch 12784183

Hi Team,

While applying patch 12784183 , following error occurs.

The following Oracle Forms objects did not generate successfully:

pa      forms/PTB       PAXTRAPE.fmx

An error occurred while generating Oracle Forms files.

Continue as if it were successful [No] : yes

Connecting to APPS......Connected successfully.

Patch was successfully applied,  we tried compiling  PAXTRAPE.fmx manually as follows, but of no luck
