HR_API_USER_HOOKS_UTILITY errors during 12.1.1 and 12.1.3 upgrades

We’re doing an 11.5.10.2 to 12.1.3 upgrade on Linux, and we’re having problems with user hooks errors in the 12.1.1 patch (6678700), the 12.1.3 patch (9239090), and a few post-12.1.3 patches. 

 

Originally there were around 50 failures, but we’ve worked through most of them using the many Metalink Notes about these errors.  We’ve applied the following patches:

 

11i Patches (we were at 11.5.10.2 + 11i.ATG_PF.H.7, 11i.AD.I.7 and 11i.HR_PF.K.7)
