Patch Advisor Fails for HRMS 11i EOY Phase 2 16000000

VPC Team member opened this thread for the customer.

The patch advisor fails for the US and Canada End of Year 2012 Phase 2 patch. 
The concurrent output log is attached. 
The InfoBundle.zip file was downloaded today from MOS and successfully uploaded to the test instance. 
The patch was downloaded today from MOS and unzipped on the server.
