OPatch: Unable to create Patch Object.

I'm trying to patch a 2 node Oracle 11.2.0.3 cluster with patch p14275605_112030_Linux-x86-64.zip.  It produces the output below.  I can't identify any additional relevant information when I turn on opatch debugging.  Patch p14275572_112030_Linux-x86-64 has been applied.  What causes the error?
