PR Approval Issue

Hi,

Most of PR stuck during approval with following error;

Requsition submitted for Approval stuck in following error;
Failed Activity : Update Approval List Response
Activity Type : Function
Error Name : -6512
Error Message : ORA-6512 : at line

All PRs will be approved after Retry from Workflow.

This is slowing down the business process.

Any help,  why it is happening and how to fix it,  is highly appreciated.
