Error in Ledger Options set up in R12

Hi ,

I encountered the error message below trying to set up th Retaind Earnings Account in Legder Options page under the Accounting Manager set up in R12 and have not been able to progress as a consequence:

'SSV.do_dynamic_insert_for_java() is unable to insert new combination. into table GL_CODE_COMBINATIONS. SQLERRM: ORA-00957: duplicate column name'.
Will appreciate any suggested solution . Thanks.
