Request new database from DBAs for sandbox environment

I would like to know if there is a "set" request to send to our DBAs for a new instance.  This would be for MS SQL and I would like to know what they need to setup before we can run the install and database wizards.  Our DBAs do not know Siebel etc. anyone have a "template"?
