Workflow- Pre field= Null

Hi,

I created 4 pick list fields on an Account record for different dept's to review & either select "Approve" or "Decline".  Then, there are text fields that update based on which field is updated that populates the "Modified By" information, but I am having a hard time with the text fields only updating when the field is modified.  Below are the following that I've tried so far.  Have any thoughts?  Am I overthinking this?

PRE('<IndexedPick0>')<>[<IndexedPick0>]  -Only updates if the field value changes from an existing value to another (not if changes from being NULL to a value).
