Partial Requisition information recorded

All,

we recently upgraded our EBS system.  View history information recorded for one of  the requisition is partially recorded with just sequence number and performer information. Date and Action information was not recorded at all. Furthermore, phased out system shows the requisiton over all status as approved whereas new EBS system shows the status as pre-approved. PLease can some one let me know how to correct the issue.
