Unable to customize 1099 Template

Hi,

I wanted to add a new field ( Vendor Address line2)  in 1099 form using  Adobe Acrobat X Pro . I could save the PDF template but BI Publisher  Template Viewer is erroring out with a error message as "[011013_100410787][][ERROR] The PDF file seems to be corrupted.  Trying to fix the Xref tables." "FO Formatting failed.".

User requirement is to have two vendor address lines instead of one line having 'address_line1||address_line2' .

How to accomplish this ?
