Attachment to a SR fails

Recently it looks like the SR attachment screen has changed - I am now unable to attach anything to an SR - it "fails" each time.  I've tried different document types and sizes.  Any suggestions?  Oracle Support will be unable to work my SR if they don't have the files requested.
