Application Failure Header Label - VM Template 912 - assistance needed

Hi All,
I finally managed to get my VM templates working, the servers start smoothly, I am able to sign on the systemand everything looks pretty UNTIL I dare to launch ANY application. When I do I get the message as per attached error_01.jpg. I do not fill any data because I simply do not know what to put in, I just click OK/Save...

...and I get the popup as per error_02.jpg

I spent so much time and lost almost all my nerves trying to get it up and running (my choice was unfortunate - OVM 3.1.1) to see such issues.
