WEB ADI fails at "Applet Not Initialized'

Hi,

One of the web adi functionality fails at view hierarchy.

Environment details:

Application version: 11.5.10.2

Database version: 10.2.0.4.0


Navigation steps to view account hierarchy:

Connect Oracle applcation front end link --> Click on Apps Logon Links --> Click on E-Business Suite Link
-->Select the Responsibility as UW_USD_GL_SYSTEM_MANAGER --> In UW_USD_GL_SYSTEM_MANAGER page, Go to setup --> accounts --> manager
It's failes at click and open the manager option, then we can view the Account Hierarchy"

AHM profiles set correctly.even through below error throws.

Error messsage : "Account Hierarchy Manager Fails To Start With 'Applet Not Initialized'
