Active Supplier Site not listed on Supplier Address Book Form

Good Morning -

We setup/added 2 new sites to a Supplier which had 3 existing sites.. (version 12.1.3).

All 5 sites are listed on the main Quick Update tab.  Yet, when we go tp the Address Book form for the Supplier - only 4 of the 5 sites displayed.  The missing site was one of the existing sites.  

I have attached screen prints.

Does anyone have any guidance as to why an active site would not appear on the Address Book form?
