Approval list could not be generated

Hi,

We are facing an issue , when user is approving the PR document he is getting approval list could not be generated. And when querying in core application form the Authorization status is in In-complete and the action history is showing as Approved.

Checked the AME flow by run Test case and Checked employee's and FND user's but all are working fine.

Please help me out of this error.

we are using R12.0.4 version .
