Purchase requisition is going to incomplete status

 Hi,
I have an issue where Purchase requisition is going to Incomplete status as soon as PR is raised and i am unable to find any lines and distribution for the associated Purchase requisition and no action history also for the PR.

we are using Iprocurement for raising PR's and Supervisor hirarchy and AME for approvals.

we are on Oracle 12.0.4 version.

Please guru's help me in solving this issue .
